i try and use templates wherever...usually driven by what the customer wants to pay.
Templates are so easy to control and when they are driven by css then usually one template can be css'd to do almost anything!
We create from scratch, however if a customer came with a prescriptive order and specifically requested a template, then that would be fine too, though it isn't standard.

Unless someone did a survey to every web busines to ask, then we are not able to give you that answer. There are plenty that do and plenty that dont. Not everyone can afford a bespoke site.

Do most web designers build from Scratch or use a template and edit it? I am just curious.
There are so many ways to create a website that I don't think you'll find one method which most web designers use.
With our web designs based on our sitebuilder, we build from empty layouts. I make the distinction between layouts and web templates, since the word 'web template' has many different meanings, different meanings to different people. Web templates can go from the simple empty ones, but they are usually more associated with the more busy designs, pre-branded with graphics and colour schemes, as in the web templates from template monster.
With layouts there is more flexibility since there is less to them originally, they have no branding, and therefore more for you to add your design and branding to, e.g. an empty space in a certain layout, 2 column, 3 column, navigation at top, navigation to side, etc.
Most websites, whatever way they created, can be categorised by certain layouts, e.g. 1, 2, 3 column, menu at top, menu to left, branding area at top, footer area at bottom, whereas most websites cannot be categorised into web templates since web templates are more specific.
