Depends on the expectancy. Some will stick at £200 a month because that is their set methodology regardless of client, while others will charge more, and hopefully do more to help the site improve quicker. It all depends on workload really.

Piece of string question. For some phrases a single hit might be enough, while for others like web design/mortgages/loans/etc a far longer campaign will be needed.
the length of the campaign is directly relevant to the competition for and the volume of keywords.
E.G. Ask a bricklayer how long does it take and how much will it cost to lay bricks, and the first thing he will want to look at is your plans. Same with SEO. If you are planning a 40 bedroom mansion it will cost a lot, a single garage OTOH won't.
The monthly fee is misleading IMHO. If you are paying £200 a month you are only getting about four hours of work unless you are using a one man band with no overheads.
If you are using a company expect to pay around £400 a day.
If you are using a one man band freelancer maybe half that.

That's less than £5.20 an hour. I assume it's an overseas company?
These guys often build hundreds of 'pretend' websites and stick your links on them. Ask them for a sample of their work and pass it to me and I'll evaluate it for you.
Well in my case Ive split the cost up, there is a bit upfront cost for optimization and some link building.
There is then an optional link building pacakge too. I think as has already been stated you need to find out exactly what you are getting for your money.
